You can create a simple icon in any editor, in Paint, for example, save it in BMP format, say, and then translate it into ICO format.

Or for example. For web pages I use icons of the same size 16X16, but for Windows systems eg. The icon should contain several drawings of a number of sizes. Here you also need a good icon editor.

Transparent background

Once transparent background is convenient, but for the best quality try to avoid it. This is simpler and using the shape of the square, you make the most of the area given to you under the icon (it's already small, why cut back). But sometime it's necessary and sometime you want.
If you create an icon in a regular editor, it will not save it in the ICO format. You will have to save it most likely in BMP and then convert it to ICO format. But the transparent background is usually replaced by black color. And you will need to remove the black color in the icon editor.

Also creating a transparent background, remember, if there is transparent, then there is no translucent. Let's say you have a red circle, then the cells will be pink on the edges because the editor draws with a transparent background, like on a white background. Then you need to predict on what background your picture will be and replace the pink with the appropriate one in your case. Or give up the smoothed edges and make everything red. In this case, the edges may look ragged.
